From Rt. 15, take Rosemont Avenue exit. Turn west (towards Yellow Springs). Go approximately 3/4 mile, pass Food Lion on your left. Pass first Old Farm Drive on left, continue approximately 1/4 mile to second Old Farm Drive. Turn left on the second Old Farm Drive. Avondale Professional Building is on your left. Drive into lower parking lot to the entrance on the lower level in the building nearest Yellow Springs Rd.

From the East, take Rt. 40 Alt. into Middletown, turn right on East Green Street.
From the West, take Rt. 40 Alt. (via Rt. 17 from North or South) into Middletown, turn right on Schoolhouse Rd then an immediate left on East Green Street.
Once on East Green Street, the office/house is at the top of the hill on the right. The driveway is adjacent to the school parking lot.
